Surrey Care Group Recognised For Outstanding Partnership With Local Community Alliance

A family-owned Surrey care group has recently been recognised for its outstanding partnership with a local alliance that aims to deliver the best and most sustainable outcomes for its communities.

The partnership between CHD Living and North West Surrey Alliance has proven to be instrumental in addressing the healthcare needs of the community. Through both of the organisations’ collaborative initiatives, there has been successful facilitation of hospital discharges and the provision of essential services to a significant number of individuals, contributing to the overall improvement of community health and wellbeing. Specifically, CHD Living has supported hospital discharges through its services with block bed contracts – which has positively impacted the lives of more than 600 individuals to date.

The North West Surrey Alliance is a partnership of diverse organisations, aiming to break down boundaries to deliver optimal and sustainable outcomes for the local community – driven by a shared commitment to support health, care, and wellbeing in the region.

Comprising a range of partners, including traditional health and care services, local councils, charities, and housing associations, the alliance leverages the strengths, knowledge, and resources of each entity to form a unified voice. This collaborative approach is aimed at creating, planning, and delivering innovative solutions that cater to the distinct needs of the North West Surrey community.

The awards not only reflected the HSJ Awards’ enduring ethos of “sharing best practice and innovating drivers of better service”, but also served as a timely and well-deserved ‘thank you’ to the sector during the 75th anniversary year of the NHS – providing an opportunity to celebrate the achievements of healthcare organisations and professionals.
