CQC Update – The End of the Single Assessment Framework is Coming!

The Care Quality Commission (CQC) is introducing major CQC framework changes in 2026, replacing Single Assessment Framework with sector-specific frameworks tailored to different areas of care.

While the 5 key questions (safe, effective, caring, responsive, well-led) will remain the same, the way services are assessed will change.

No immediate action is required — changes are expected from mid-2026, with a full rollout later in the year.

What Is Changing in the CQC Framework?

Following the consultation “Better regulation, better care”, CQC is moving away from a single framework towards more targeted, sector-specific approaches.

CQC has developed four draft assessment frameworks for:

  • Adult social care
  • Mental health care
  • Primary care and community services
  • Hospitals (secondary and specialist care)

This shift is designed to make inspections more relevant, specific, and meaningful for each sector.

What Is Staying the Same?

The 5 key questions will remain central to all CQC assessments:

  • Safe
  • Effective
  • Caring
  • Responsive
  • Well-led

These continue to form the foundation of how services are evaluated.

What Will Replace the Current System?

The new frameworks introduce two key changes:

1. Key Lines of Enquiry (Updated Approach)

  • Structured questions explaining what CQC will look for
  • Replacing the current quality statements
  • Designed to give clearer expectations for providers

2. Rating Characteristics

  • Clear descriptions of what Outstanding, Good, Requires Improvement, and Inadequate look like
  • Tailored to each specific sector
  • Helps providers understand how ratings are judged

Important: Scoring will be removed under the new approach.

When Will These Changes Happen?

  • Changes expected to begin: Mid-2026
  • Full rollout: November / December 2026 (expected)

For now: carry on as usual.
There is no requirement to make immediate changes.
